The below information is regarding the collection, usage and storage of your personal data, your rights regarding personal data and conformity to legal requirements for data protection. Should you wish to proceed with counselling you will be asked to sign a consent form, view and agree to a counselling agreement about shared obligations for therapeutic work and confirm you are okay with the collection, storage and usage of your personal data.
Any and all data is held in compliance to the General Data Protection Regulation (GDPR) 2018 with any electronic/digital copies of personal information being encrypted and password protected and physical copies anonymised via codes before being stored in a locked cabinet. I am the only person with access to the digital files and locked cabinet combination. I only collect data pertinent to our therapeutic work together and securely dispose of your personal information 1 year after our final session together with any notes destroyed a maximum of 7 years after our final session together.
You have the right to request your data at any point and amend it should there be incorrect information held. You also have the right to withdraw consent at any point where there is no legal duty for information to be retained or request to erase your personal information that I hold.
You have the right to request that your information be transferred to another party.
At SL Counselling and Psychotherapy the data controller is Sarah Laycock in accordance to the GDPR regulations, 2018. I am fully registered with the Information Commissioners Office (ICO) for data processing.
